We report on a case of an unexpected dissection of the left main stem during percutaneous coronary intervention complicated by involvement of the ascending aorta with pericardial tamponade. After pericardial drainage, haemodynamic stabilization, and extensive stenting of the propagating dissection, safe discharge was possible without surgical intervention. Abstract
