PREFACE 2end Edition. Following the success, five years ago, of the first edition of the clinical handbook "Challenging Cases of Back and Low Back Pain", it is with great pleasure that I worked on the secon edition of the book. The chapters contained in this new edition are written by group of world spine experts, among which some had already participated to the first edition. Nevertheless, the philosophy of the book remained the same: to provide the reader with a mix of adult and pediatric clinical cases on various aspects of back pain and its treatment. The hope being that the chosen format will help the readers to better understand the problems related to several spine desorders and the principles of their treatment. On a personal note, special thanks to Shveta Dhamjia Khera and Tarun Choudhry for their help, diligence and efforts to bring this updated second edition into being. We hope you will enjoy this second edition as much as you did the first one! Federico Canavese
