Introduction. Atypical Fibroxanthoma is anuncommon pseudomalignant tumor of mesenchimal origin that occurs primarily in the elderly after the skin has been damaged by sun exposure or ionizing radiation. The head and the neck are predominantly involved. The treatment is surgical; the tumor removed radically has good prognosis with scarce tendency to the relapse. Case Report. We report the case of a 74-year-old man, radiodiagnostic technician from 1964 to 1993, assigned to X-ray fluoroscopic examinations of industry workers on board of Mobile Units. The technician had frequent exposures close to the patients, worked until 12 hours per day and didn’t use protective equipments. About 30 years ago skin lesions of the vertex appeared, that have progressively been infected and ulcerated since 2006. A biopsy of the lesions lead to the diagnosis of Atypical Fibroxanthoma of the scalp. The patient underwent surgical excision. Discussion. The occupational exposure to ionizing radiation has lasted 29 years; in the first 19 years of activity an annual mean exposure of about 50 mSv/year was recorded. The latency between the beginning of the exposure and the onset of skin lesions is 18 years; the tumor has appeared after other 24 years. The quantity and duration of exposure are compatible with the diagnosis of chronic radiodermatitis and cutaneous tumor induced by ionizing radiation
