Reconstruction of the scalp after tumor resection, trauma, alopecia or after irradiation represents a challenge for the plastic surgeon where the reconstructive ladder plays as surgical guide for the coverage planning. however, the biomechanical properties of the scalp and its tensile behaviour due to the presence of a tendon-like structure like the galea aponeurotica must be considered in order to avoid wound closure sequelae. Scalp reconstruction is both for functional and cosmetic purposes: hair restoration must be considered as secondary step to restore cosmetic appearance of the scalp or adjust anterior hairline, for camouflage of incisions lines or for eyebrows reconstruction.
